Let’s Talk About Dying: “Advanced Care Planning–Making Sure My Wishes Are Followed”

CCH is so honored to welcome Michelle Cantillo, RN, who will be our leader through three important conversations about death and dying. 

During this third conversation we will learn the difference between an advanced directive and POLST, Michelle will be available to notarize, and we will briefly touch on estate planning.

Michelle Cantillo, RN, is Advance Care Planning (ACP) Coordinator for Hawai‘i Pacific Health (HPH). Her passion for advance care planning stemmed from working in critical care over 30+ years and being at the bedside of dying patients. Michelle’s role now includes navigating patients and providers thru Hawai‘i’s law on Medical Aid in Dying, Our Care, Our Choice Act.

During this second session, titled “What Matters to Me,” we will learn about the differences between hospice and palliative care as well as how to begin planning for decision making using a work book.

The first session with Michelle is called “Having the Conversation” and we will talk about how to discuss the “d-word” in different contexts. To aid our discussion we will play a game called “Go-Wish.”

Let’s Talk About Dying

It’s Lent, and we’re thinking about what it means to come face to face with our own dust, our own humanity. Together we’ll have the opportunity to learn and talk with each other about critical issues every human must confront.  Register to join us by calling the church office, clicking on the website link, or signing up with a volunteer in Aloha Hour on Sundays.  Friends and family are welcome and encouraged to attend.  If you need childcare, please let us know and we will work to accommodate your needs. CCH is so honored to welcome Michelle Cantillo, RN, who will be our leader through three conversations: “Having the Conversation,” “What Matters to Me,” “Advanced Care Planning—Making Sure My Wishes Are Followed.”

Spiritual, medical, legal, financial issues we need to think about but often avoid will be part of our conversations over these four weeks. In addition to opportunities to learn, we’ll have access to resources like legal documents, advanced care planning forms, the services of a notary, resources for writing and executing final wishes and a last will and testament, along with much more.
