CCH is so honored to welcome Michelle Cantillo, RN, who will be our leader through three important conversations about death and dying. 

During this second session, titled “What Matters to Me,” we will learn about the differences between hospice and palliative care as well as how to begin planning for decision making using a work book.

Michelle Cantillo, RN, is Advance Care Planning (ACP) Coordinator for Hawai‘i Pacific Health (HPH). Her passion for advance care planning stemmed from working in critical care over 30+ years and being at the bedside of dying patients. Michelle’s role now includes navigating patients and providers thru Hawai‘i’s law on Medical Aid in Dying, Our Care, Our Choice Act.
